What this prototype is, how it is grounded, and what a production version would require.

Demonstration prototype — synthetic data only

Infuse is a demonstration prototype, not a clinical system. Every patient, NHS number, record and note is fabricated. It does not connect to any real EPR, NHS API, or social-care system (such as WCCIS). It is not for clinical use.

What Infuse is

Infuse sits between hospital discharge and social care. It captures discharge information, structures it with AI against a standard dataset, matches the patient to the Newport City Council Adult Services record (WCCIS) with human confirmation, analyses completeness, reconciles against what social care already holds, and routes a precise gap request back to health or prepares verified data for write-back after sign-off.

PRSB standard basis

The dataset is a credible simplified model of the PRSB “Hospital Referral for Assessment for Community Care and Support” standard — the NHS-owned successor to the retired SCCI2075 notices. It is a demonstration model, not the full published standard.

Welsh context

Set in Newport, Infuse shows Aneurin Bevan University Health Board discharging a patient into Newport City Council Adult Services, recorded in WCCIS (the Welsh Community Care Information System) and written to Mosaic. It reflects the Discharge to Recover then Assess (D2RA) model with Pathways 0–3 and includes an English / Cymraeg toggle to demonstrate bilingual intent (the Active Offer).

Infuse feeds Mosaic

Infuse is the AI intake, triage and collation front-end that hands a clean, structured referral to Mosaic — the local authority's case-management system of record. Infuse captures the governance a social worker needs and hands off; it does not rebuild Mosaic's statutory assessment, eligibility or authoring.

Designed for safety

  • Human in the loop

    Identity matches and write-backs require explicit human confirmation. Nothing is written silently.

  • Grounded, not invented

    Extraction marks anything not found as absent. The gap ruleset is deterministic and explainable in both demo and live modes.

  • Accessible by design

    Status is always carried by icon and text, never colour alone, targeting WCAG 2.2 AA.
